Rapid Response referral process; all health and social care professionals can refer in to the Rapid Response service including;

Members of the Public can contact Bury Adult Care Connect and Direct Hub direct on 0161 253 5151 (CADHUB ) where customer service advisors will screen the referral and determine if a Crisis - Rapid Response is required, and direct to the Rapid Response Team if requires a 2 hr response’ is required.
Members of the public can contact there GP or any other social care or health professional to refer them direct’. If a Rapid Response is not required, CADHUB will refer to ACS Locality Social work teams, for awaiting assessment.
