The specialist palliative care team are there to listen, provide supportive care, advice and information to patients, their families and their carers. Working with other health and Social Care professionals we aim to manage symptoms that may arise to enable patients to experience the best quality of life.

We also provide education and are a specialist resource to other health care professionals. The team work as part of the Specialist Palliative Care Multi-Disciplinary Team that includes the Fairfield Hospital Macmillan Nurses, palliative care consultants, dietitians, Occupational Therapists, physiotherapists, social workers, carers support workers, Crossroads and the Bury Hospice Inpatient and Day Care team.

We offer advice and support both directly to patients to enable them to maintain the highest quality of life and also indirectly, through generic community services, particularly GPs and District Nurses, to support palliative care patients in their own homes. Helpline available Monday to Friday 9am to 5pm.
