Details
The Bury Health Visiting Service includes a small team of SEND Health Visitors who carry a smaller caseload to support children with additional needs and their families. Referrals for caseload cases are taken solely from the Bury Health Visiting Service.
The Health Visitor's also provide training, consultation and a 'step-on' referral process to support the care provision for children with additional needs within the wider service. They also work closely with other local teams such as Portage and Community Paediatrics to support children to access local services. The team can also support children to access to local Early Years educational settings and opportunities to prepare them for the transition to full-time education.
The SEND HV Team also work closely with the Enhanced HV Team who can provide an 'enhanced' visiting schedule for families with a wide range of complex needs (referral to this team are open via Midwifery, Bury Parent Infant Mental Health Service (PIMHS) or the Health Visiting Service).
