Bury community mental health service

We provide support for people aged 16 to 65, with severe and enduring mental health problems and complex needs, who present significant levels of risk to themselves or others.

This includes people with severe and complex:

  • Symptoms of depression
  • Symptoms of anxiety
  • Psychological difficulties, including personality disorder
  • Psycho-social needs
  • Psychotic symptoms

We are a multi-disciplinary service made up of managers, community mental health nurses, mental health social workers (some qualified as AMHPs), Occupational Therapists, community support workers, admin support, secondary care psychological therapies and Consultant Psychiatrists who work with the team.

The service is made up of five key functions:

  • Assessment
  • Active care coordination
  • Treatment
  • Reviewing officer
  • Wellness and recovery

You will have an identified care coordinator or key worker dependent upon your level of need.

This practitioner will be responsible for supporting you to develop a care plan to meet your needs, following a comprehensive Assessment.

They will also be responsible for regularly reviewing the care plan and supporting you to prepare for discharge from the service, as and when appropriate.

Referral Notes

Professionals can refer to the service by contacting the Access Team which provides the single point of entry for all adult mental health services in Bury (Tel: 0300 200 3440). The service is unable to accept referrals from non-professionals.

Referrals for an assessment under the Mental Health Act should be made directly to the duty AMHP by telephoning 0161 253 7997 or out of hours to the Emergency Duty Team by telephoning 0161 223 6606.
