IMPORTANT INFORMATION

WE ARE NO LONGER SEEING PATIENTS IN CLINIC

All face to face appointments have been cancelled. Please do not attend any appointments with your child.

You will receive a letter in the next week with further information. We are making every attempt to contact all families to discuss each child's specific needs.

If you need to speak to a member of the Team please ring one of us to discuss (see ‘who to contact’ for a list of clinic telephone numbers).

Please explore this page for further information, advice and activities relating to  speech, language and communication development.

Referral Notes

Requests for assessments can be made by parents, GPs, Health Visitors, School Nurses, Paediatricians, Special Educational Needs Coordinators or other professionals involved with the child.

The ‘request for assessment’ forms can be found in the download section. Please note there are separate forms for preschool and school aged children with speech, language and communication needs. Please ensure all sections are completed.  The information you provide will help our understanding of the child’s unique skill set and how their speech, language and communication needs are impacting on their daily life.

If you are concerned about a child’s speech, language or communication development but unsure about completing a ‘request for assessment’ form, give us a ring and speak to any member of the Speech and Language Therapy Team (see ‘Who to contact’ above)
