Due to the ongoing situation regarding COVID-19 and the required social distancing measures, the Speech and Language Therapy Service continues to work flexibly in order to provide the best support for you and your child.
If you have any questions regarding your child's Speech and Language Therapy support, please contact your Therapist whose details can be found on your most recent care plan and/or appointment letter.
If you are unsuccessful reaching your Therapist you can contact our Team Leader Kirsten Brooks (see contact details on this page) who will ensure your message is passed on.

Requests for assessments can be made by parents, GPs, Health Visitors, School Nurses, Paediatricians, Special Educational Needs Coordinators or other professionals involved with the child.
The ‘request for assessment’ forms can be found in the files section below. Please note there are separate forms for preschool and school aged children with speech, language and communication needs. Please ensure all sections are completed. The information you provide will help our understanding of the child’s unique skill set and how their speech, language and communication needs are impacting on their daily life.
