SLS are a specialist care provider that supports individuals with learning disabilities, Autistic Spectrum Conditions, sensory processing difficulties, learning difficulties or complex health needs to live in their own home and access the community, while being supported by a team of well trained and committed staff. SLS follows the principles of Positive Behaviour Support and Active Support, this means we never make the asssumption that someone can't do something, we support people to be independent and do as much as they can for themselves.
SLS will support and advise individuals and their families through the Transition process of moving into their own home for the first time, alternatively SLS will work in partnership to create a flexible support package for people requiring support in their current home, or if you are looking to change your current support provider. This may mean supporting someone 24 hours a day 7 days a week or providing support for a number of agreed hours each day, increasing to full time support, we understand the importance of getting it right from the start.
We can offer help with:
- How to access support if you currently don’t receive any.
- Deciding the best way to use your money, whether this is through a direct payment or individual service fund (ISF).
- Transition planning if you are moving into your own home for the first time or from another property, this may need to start with a few hours a week building to up to full time support if that’s what is needed for the individual being supported.
- Provide guidance on how to access benefits including Personal Independence Payment (PIP), Employment Support Allowance (ESA), housing benefit, Universal Credit etc.
- Support you with finding the correct property to meet your needs and understanding what adaptations may be required to make a property suitable.
- Support planning while working in partnership with individuals, their families, Local Authority and health professionals, to ensure there is a clear person centre support plan in place that has clear short and long term goals agreed.
- Networking with other individuals and families receiving or looking for support in their own home.
