The EHC Assessment Application

Schools or colleges requesting an EHC assessment must do so by completing the relevant application form, which can be found in the 'related files' section. Please note that the application form contains clear guidance notes as an appendix.

The application needs to identify outcomes for the child/ young person and any reports should highlight these outcomes. The referral should be accompanied by evidence of provision made for the child/ young person from the SEN Notional Funding, typically over three terms. This must take the form of a costed provision map. The parent/carer and the child/ young person’s views must be submitted with the application.

Settings should make every effort to identify strengths in all areas as well as difficulties. It is also helpful to detail what has worked in previous cycles of assess-plan-do-review and what additionality is needed to maintain or enhance the rate of progress.
