If a child/young person is within 12 months of transferring from nursery to primary school or primary to secondary school, a review needs to be held to inform this transition.  Any changes to the EHCP should be made by 15^th February of the calendar year in which they are due to transition.

If a child/young person is transitioning from secondary school to post-16 education (including an apprenticeship), a review should take place by 31^st March of the calendar year in which they will transition to post-16. As described above, a clear focus on preparing for adulthood needs to be included in all Annual Reviews from Year 9 onwards.
