What is a care needs assessment and how do I get one?
If you have tried other ways to stay independent and still need further support, the first step is to contact us for a conversation. We may suggest that you have a social care needs assessment.
A social care needs assessment is a discussion with you, relatives, or friends who know you well, and look at your strengths, your needs and will consider your personal wishes in deciding what care, community support, advice or information you need. The assessment may be over the telephone or face-to-face.
You have the right for an assessment no matter what your ability to pay for the care or services you may need and you have the right to receive a copy of your assessment and how your eligibility was established.
You can be referred for a social care needs assessment by:
You or your relative, friend or neighbour
A social worker or health professional such as a doctor, nurse or health visitor
Another agency
Adult Care Connect and Direct is the first point of contact for adults (18+) who need an initial assessment or response from Adult Social Care Services.
