If you are eligible for help, we will work with you to develop a care and support plan. A care and support plan sets out how you can get the right level of support so that you can do the things that are important to you.

Everyones’ needs for care and support are different and needs can be met in many different ways.

We will try to do this through the support of your friends and family or services in your local community wherever possible and will give you information about other  kinds of support available that can help you, such as support groups.

Once the support plan has been developed and agreed, we will review the plan to make sure that your needs and outcomes continue to be met over time. If anything has changed, we will carry out a new assessment.  You can also request a review of your care and support plan.
