Adult Learning Disability Team, Bury

We support people with mild, moderate or severe learning disabilities who are registered with a Bury GP, to help them live a valued life in their community.

Our team is made up of Learning Disability nurses, Occupational Therapists, speech and language therapists and psychologists.

We deliver a specialist Assessment and intervention plan, which focuses on health inequalities, behaviour, risk, sensory, communication and psychological needs.

We work in partnership with families and agencies to achieve positive outcomes for you in your home, community and other relevant settings.

We accept referrals from service users, carers, social workers, GP's and other health professionals. All referrals will go through a screening process which checks whether you are eligible to access the service.

In previous years, our colleagues have picked us as winners of the Principles of Care Awards, for our work with Down's Syndrome and dementia patients.
